- - The Centers for Disease Control and Prevention (CDC) teamed up with the National Association of City and County Health Officials (NACCHO) to develop the new Million Hearts® in Municipalities Toolkit, a tool to help health departments and municipal organizations implement Million Hearts® at the local level. It was designed to engage city, county, and regional partners to adapt the Million Hearts® structure and framework to the needs and assets of their community. It includes 5 modules that can be used, individually or as a package, to tackle capacity needs related to goal-setting, partnerships, communications, and evaluation and monitoring. Each module includes a list of online resources that may be used to inform the design of a municipal’s evidence-based approach to prevent heart attacks, strokes, and other cardiovascular events. We encourage you to explore how this tool may be used to inform your cardiovascular disease prevention efforts.
